The following is a summary of “COVID-19 adaptations to a training and support programme to improve primary care response to domestic abuse: a mixed methods rapid study,” published in the January 2024 issue of Primary Care by Downes, et al.

The COVID-19 pandemic has brought about an increased incidence and reporting of domestic abuse (DA). Lockdowns and social isolation necessitated innovative ways to support victims remotely. Identification and Referral to Improve Safety (IRIS) is a program enhancing the response to domestic abuse in general practice, proven effective and cost-effective. 

For a study, researchers sought to explore the relevance, desirability, and acceptability of adapting IRIS to operate remotely during the pandemic.

The mixed-methods rapid research involves triangulating quantitative IRIS referral data with insights from four surveys and 15 interviews. Participants include local IRIS teams, IRIS-trained clinicians, and victim-survivors supported by IRIS services. The Lean Impact approach facilitates swift evaluation of innovation and social intervention impact. Framework analysis of interviews provides a qualitative understanding of findings.

The adaptation to online training and support for IRIS was deemed acceptable and desirable. Clinicians expressed confidence in addressing DA remotely, although face-to-face interactions instilled greater confidence. Although referrals initially declined in March 2020, they rebounded to pre-pandemic levels by July 2020. Patients reported feeling well-supported remotely, though those with previous face-to-face experience preferred in-person support. Technology emerged as a prominent barrier during the transition to online training and remote support.

The study emphasized the desirability and acceptability of training clinicians to identify, inquire about DA, and refer to the IRIS program during telephone/online consultations. The analysis provided valuable insights for health and public health commissioners in making commissioning decisions to enhance general practice responses to domestic abuse, particularly in the context of remote support.
