Approximately 25% of ESRD patients will be placed on the kidney transplant waiting list. Almost 15% of these individuals will require retransplantation operations. For a study, researchers tried to synthesize the most recent knowledge on many difficult topics concerning retransplantation in order to offer the readers a clear and up-to-date perspective on the subject. Despite the fact that most current research is based on retrospective, limited, single-center experiences, it became obvious that retransplantation remained a surgical and immunological challenge for which perioperative treatment was critical to minimize complications. Various surgical methods have been tried, but the general opinion favored heterotopic extraperitoneal in the first instance. 

Although the existing series consistently reflect increased immunological risk and complication rates, the advantages in terms of overall survival are superior to dialysis, making it the most recommended alternative for a group of patients considered in the study.
