The following is a summary of “Chronic kidney disease and adherence improvement program by clinical pharmacist-provided medication therapy management; a quasi-experimental assessment of patients’ self-care perception and practice,” published in the December 2024 issue of Nephrology by Mohammadnezhad et al.
Pharmacists play a vital role in community and hospital settings by providing medication therapy management (MTM) services.
Researchers conducted a retrospective study to assess the impact of clinical pharmacist-provided MTM on patients with chronic kidney disease (CKD).
They conducted a cross-sectional survey from June 2023 to January 2024 using a validated questionnaire to assess patients’ knowledge, attitude, and performance. The Morisky medication adherence scale measured adherence before and after clinical pharmacist-provided MTM in patients with CKD and GFR < 60 mL/min/1.73 m2, as per NKF KDOQI guidelines.
The results showed a significant improvement in all outcomes (P < 0.05). A total of 100 patients completed the questionnaire. Knowledge increased from 3.38 ± 1.82 to 7.59 ± 1.11. The attitude score improved from 54.53 ± 5.05 to 59.24 ± 4.68. Performance score increased from 30.15 ± 4.27 to 34.54 ± 2.56. Adherence score rose from 4.86 ± 1.99 to 6.22 ± 1.39.
Investigators highlighted the key role of clinical pharmacists in improving medication adherence through education, showing positive outcomes from pharmacist-provided MTM services.
