The following is a summary of “Trends in price for topical corticosteroids from 2017 to 2021 and the opportunity for cost savings identifiable at the point of care: A retrospective cross-sectional study,” published in the January 2024 issue of Dermatology by Kwa et al.
Despite numerous generics and substitutes, the cost of topical corticosteroids can be a barrier to treatment for some patients. Researchers conducted a retrospective study investigating recent trends in pricing for topical corticosteroids and assessing the potential for cost savings associated with them.
A cross-sectional analysis was conducted on all prescriptions dispensed for topical corticosteroids (January 1, 2017, to December 31, 2021). Data sourced from US all-payer pharmacy claims with commercial coupon database.
The results showed 237 distinct drug products with ≥1 claim. Branded products cost 105% more than generics, highlighting a substantial price gap. Ultrapotent topical corticosteroids were 55% pricier than those of low potency. Ointment formulations were found to be 19% more cost-effective than creams. While cash prices for most products remained relatively consistent over the 5 years, a significant 63% increase was observed in the prices of branded ultrapotent topical corticosteroids. Switching from branded to generic options could save $14.75 per unit, and transitioning between generic versions could save $6.82 per unit. The data indicated that coupon prices consistently reflected lower costs compared to cash prices, with a correlation coefficient (r) of 0.89, showcasing the reliability of coupons in estimating real-time expenses. Excluding negotiated insurance rates limited the study’s cost analysis.
While the pricing landscape for topical corticosteroids has largely remained stable over the past 5 years, exceptions include, notably, the rising costs associated with branded ultrapotent formulations. Significant cost-saving opportunities exist despite these fluctuations, especially when transitioning between similar-strength generic alternatives. Furthermore, coupon prices provided a consistent and reliable reflection of costs, aiding patients and healthcare providers in making informed decisions about treatment affordability.
